Disease Management and Wellness services can have a variety of benefits to employees but are also helpful to the employer. Chestnut Global Partners' (CGP) can have a positive impact on employee attendance, productivity, and workplace engagement and satisfaction. Additionally, our programs can decrease claims cost, worker's compensation costs and workplace accidents/injuries.   
 
Each of our programs are customized to measure the outcomes and return on investment that is important to the employer. Additionally, and when available, CGP can integrate our program results with employer statistics such as actual claims cost, attendance and others through scientific data analysis.
 
 
Disease Management Services 
 
CGP's health coaches provide frequent and proactive contact with participants who enroll in disease management services. Disease management modules include diabetes, high risk for diabetes, heart disease, and depression.
 
 
Health Risk Factors
 
CGP's health coaches provide services to those employees who are dealing with single risk factors such as blood pressure, cholesterol, weight, smoking, or lack of physical activity. The goal of working with employees at this level is to prevent the worsening of health concerns and the possible onset of disease states.
 
 
Training and Education
 
Workshops, training, materials, and education is available on a variety of topics ranging from physical activity to healthy eating to improved resilience and mental health.
 
 
Health Risk Assessment
 
CGP is able to offer the employer a Health Risk Assessment (HRA) which is beyond what is typically seen in the market. Our HRA is capable of maintaining an employee history and actually designs itself around the profile of that employee. Unlike traditional “off the shelf HRAs” our HRA will recall an employee’s age, prior answers, and other information to create an HRA experience that is truly personalized to the individual employee. 
 
CGP’s HRA can also be integrated with other data sets such as claims or productivity in order to better show the impact of health on employer concerns.
