 Provider Application Process
 
 
Chestnut Global Partners (CGP) is constantly working to improve our network of clinicians and the locations we can serve.  We are committed to providing the highest quality of care to our members. We appreciate your desire to join our network and look forward to further interactions with you.
 
To begin the process of joining CGP's domestic network of providers and affiliates, you must meet certain criteria.  
 
 
Provider Application Process
 
Providers are credentialed in accordance with our credentialing criteria and with specific criteria required by applicable regulatory agencies or client companies.  
 
To be a provider in our network, you and/or your organization must be credentialed and contracted with CGP.   Once you become a provider in our network, you will need to periodically have your credentials reviewed through the re-credentialing process in order to remain in the CGP network. 
 
Provider applicants must submit the following documents in order for us to consider their application: 
  • Completed, with signature, an Individual Provider Application;
  • Copies of current certifications and licenses;
  • Documentation of education, training, and work history;
  • Documentation of professional and general liability insurance coverage and any other pertinent insurance information.
Please note that although you may complete an application, licensed providers may only be needed in certain geographic areas or be licensed for specific services.
